50 Hours Affidavit & Driving Log

Before you can take your road test in Ohio, the state requires proof that you have completed 50 hours of supervised driving practice. This practice must be done outside of driving school hours with a parent, legal guardian, or another licensed adult. Out of those 50 hours, at least 10 must be at night.

Important: By law, you cannot drive more than 4 hours in a single day toward your 50-hour requirement. This rule is in place to make sure you practice safely without fatigue.

To meet this requirement, you’ll need to:

Download the 50-Hour Affidavit & Driving Log (click the link below).
Record your driving hours on the log.
Once you’ve completed all 50 hours, have the parent, guardian, or supervising adult who trained you to sign and notarize the affidavit.

The completed and notarized form will then be submitted as part of your licensing process with the BMV.
